Jeremy Blake Collins, 29, of Muldrow, Oklahoma passed away Wednesday, February 10, 2016 in Fayetteville, Arkansas. He was born October 21, 1986 in Ft. Smith, Arkansas the son of Joy (Brawdy) Crain and Jerald Grant Collins. He was a member of Evangel Temple in Ft. Smith, Arkansas.

Jerald Collins of Van Buren, AR
3 brothers: Logan Crain of the home, and Austin and Derrick Collins of Van Buren, AR
Grandmother: Joyce McGehee of Muldrow, OK
Grandfather: Ron Brawdy of Pennsylvania
Grandparents: Jerry and Roberta Crain of Skiatook, OK
Aunts and uncles: Tim and Angie Brawdy, Joey and Robyn Haley and Greg Collins
He was preceded in death by 1 brother: Connor, grandparents: Cheek McGehee, Betty Collins and Charles Collins and uncle: Kelly Collins
Funeral service will be Monday, February 15, 2015 at 10 am at Trinity United Methodist Church in Muldrow, Oklahoma. Burial will be at Blackjack Cemetery under the direction of Agent Mallory Martin Funeral Home in Muldrow.
Viewing will be Saturday 10 am - 8 pm and Sunday 2 - 8 pm at Trinity United Methodist Church in Muldrow. The family will greet Sunday from 6 - 8 pm at the church.
