David Wayne Chuculate was born August 7, 1943 in Tahlequah, Oklahoma, the son of Levi Chuculate and Lila Jumper Chuculate. He was married to Terry Susan Campbell Chuculate on January 2, 1998 in Ft. Smith, Arkansas. Mr. Chuculate passed this life on June 16, 2011 in Vian, Oklahoma after having attained the age of 67 years, 10 months and 9 days.
